The importance of preoperative deviation on the effectivity of horizontal rectus surgery was evaluated. 123 patients
with acquired comitant esotropia and 36 with comitant esotropia; those who had only horizontal rectus surgery, followed
up for at least 2 months and had at least 2 postoperative measurements were included in this retrospective study. The
ratio of the difference between the postoperative and preoperative deviations (degree) to the total amount of recession
and resection (mm) was taken as the surgical effectivity. Mean surgical effectivities were 1.2 and 0.9 for comitant
esotropia and exotropia respectively. It was shown that in both groups of patients increasing amount of preoperative
deviation not only proportionately enhanced the surgical effectivity but also played a major déterminent role in surgical
