SUTURE IN CASES WITHOUT POSTERIOR CAPSULAR SUPPORT
In cases without posterior capsule support we evaluated the graft transparency and visual outcome following
scleral sutured intraocular lens (IOL) implantation together with penetrating keratoplasty(PK). We performed PK and
scleral sutured IOL implantation to 14 male, 4 female at Çukurova University Medical Faculty between June 1994-
January 1995. The age of cases ranged from 6 to 84 (mean 36), mean following period was 6,4 mounth. Corneal
pathology was aphakic bullous keratopathy in 3, pseudophakic bullous keratopathy in 4 cases. We performed PK,
lens extraction and scleral sutured IOL in the same session to 4 cases who had anterior chamber IOL and
pseudophakic bullous keratopathy. 11 cases was diagnoised as posttravmatic corneal leucoma. Of these, 8 were
aphakic 3 were travmatic cataract without posterior capsule.
Graft was transparent 16 of 18 in cases whose scleral sutured IOL implantation and PK was performed at the same
session. In one of the reamining 2 cases immunologic graft rejection developed and graft failure due to postoperative
endophtalmitis developed in the other case. Visual acuity was below 1/10 in 13 and above 1110 in 3 cases.
The examination of cases with transparent cornea and low visual acutiy revealed seconder glaucoma in 5,cystoid
macular edema in 2, maculopathy in 2, irregular astigmatism in 4 cases. In cases with seconder glaucoma intraocular
pressure was regulated with medical therapy in 2 and with mitomycine combined trabeculectomy in 3 cases.
Spectacles, contact lenses and anterior chamber IOL has too many disadvantages in cases without posterior
capsular support following PK. Regarding these, scleral sutured IOL implantation during PK can be an alternative
