In this study, 27 patients who underwent extracapsular cataract extraction (ECCE) with in-the-bag intraocular lens (IOL) implantation and 11 patients who underwent phacoemulsification with in-the-bag implantation combined with capsulorhexis were evaluated. Mean follow up was 28 weeks (range, 26 to 41 weeks). Best-corrected visual acuity of 5/10 or better was achieved in 78.94 % (30 eyes) of patients postoperatively. The most common 23.68% (9 eyes) intraoperative complication was radial tear of anterior capsule. At the end of mean follow up period, capsular fixation was keeping its condition in 78.94 % (30 eyes), while one haptic shift to the sulcus in 7.89% (3 eyes) and decentration among 1 mm to 2 mm in 15.78 % (6 eyes) were detected. Postoperatively, posterior capsule opacification in 7 eyes (18.42%) and capsule contraction syndrome in 1 eyes (2.63%) were determined and Nd:YAG capsulotomy was performed in these cases. We concluded that CCC which is an increasingly popular method of anterior capsulotomy, especially with the transition to phacoemulsification surgery is playing an important role of in maintaining of IOL centration and can also be used with ECCE surgery. Keywords: Capsular bag fixation,
