Ocular Hypotony After Trabeculectomy With Mitomycin C

AD, TRABZON Although success rate of trabeculectomy in glaucoma patients are reported to be higher, this operation isn't complication free (1). One of the major problem which is mostly seen in early postoperative phase is hypotony. The rate of hypotony in early postoperative course has been reported between %8-%20 of patients. It has also been reported to occur with the adjunctive use of antimetabolites, such as mitomycin C and 5- fluorouracil. Because Mitomycin C reduces the resistance to aqueous outflow normally provided by postoperative subconjunctival fibrosis, we would expect to see more frequent hypotony after trabeculectomy with Mitomycin C than after standart trabeculectomy (8).
To evaluate the incidence of hypotony in trabeculectomy with intraoperative Mitomycin C use, we prospectively compared the two different concentrations of Mitomycin C administration on eyes of 18 patients with different glaucoma types. We examined the patients in two groups. In the first group Mitomycin C concentration was 0.4 mg/ml in all eyes and treatment time was 5 minutes. In second group Mitomycin C concentration was 0.2 mg/ml in all eyes and treatment time was 2 minutes. We defined hypotony as intraocular pressure 6 mmHg and lower on two examinations at least four weeks apart and six weeks or more postoperatively. Hypotony occurred in 3 eyes in the first group and in no eyes in the second group.
