This study is designed to evaluate the results of the patients who underwent tenotomy because of overaction of inferior oblique muscle. 55 eyes of 43 patients with primary (16 of them), secondary (37 of them) inferior oblique overaction underwent tenotomy have been included in this study. Tenotomy was performed unilateral to 31 patients and bilateral to 12 patients. While there were full correction in 44 eyes, in 6 eyes grade (+), in 5 eyes grade (2+) residual inferior oblique overaction was observed in the postoperative period. Grade (+2) residual IOOA developed patients underwent the 2nd tenotomy. None of the cases developed grade (3+) or (+4) IOOA. Success rate was 80%, and 89.1% after the first and second operation respectively. This study showed that tenotomy is an effective and reliable procedure in the IOOA.Keywords: Inferior oblique overaction,
