Limbal Conjunctival Autograft Transplantationin Primary And Recurrent Pterygium Surgery

In this study, the value of limbal conjunctival autograft transplantation in cases with primary and recurrent pterygium is emphasized. The procedure was performed in 33 eyes of 27 patients. The pterygium was microsurgically dissected from the cornea, limbus and adjacent sclera and removed. All redundant cicatricial tissues over the cornea, limbus and the sclera is carefully resected. A free conjunctival graft which contained the transitional conjonctival-corneal area was grafted to the scleral bed. Care is taken to place the epithelial surface of the graft up and its limbal area close to the sclerocorneal limbus of the host bed. No recurrences developed in 19 eyes with primary pterygium, but there were 4 recurrences in 14 eyes with recurrent pterygium. Reoperations with the same technique were performed in these 4 cases resulting in recovery of these lesions. General recurrence rate was 12.1%. We noticed that recurrences developed in young patients who had previously undergone multiple excisions. No complication developed during the procedure and the follow up period. We concluded that the transplantation of the limbal area along with its pluripotential stem cells serves to regenerate the corneoscleral limbus. Limbal autograft transplantation is a good alternative treatment modality in pterygium surgery with high success and low complication rates.Keywords: Pterygium, recurrence,
