With visual inattention, or with cover testing, the nonfixating eye slowly elevates, extors, and abducts. Both eyes are usually affected but the involvement is often asymmetric. It often accompanies with congenital esotropia but can also be seen in association with other forms of strabismus. DVD should be distinguished from other types of vertical deviations. Most of the surgeons prefer to recess the superior rectus muscles. Unilateral surgery may be indicated if DVD manifests spontaneously in only one eye. The size of the DVD is useful guide in determining the amount of muscle surgery that is required. Keywords: Dissociated strabismus,
