The Effects Of Clear Corneal And Corneoscleral Tunnel Incisions On Postoperative Astigmatism And Visual Acuity After Phacoemulsification

In this study, surgically induced corneal astigmatism and visual prognosis were evaluated in cataract patients after the phacoemulsification and foldable acrylic IOL implantition surgery that was performed by using selfsealing small clear corneal or sclerocorneal tunnel incisions randomly. The same surgical procedure was performed in 21 eyes through a 4 mm clear corneal tunnel and 23 eyes through a 4 mm sclerocorneal tunnel incision. All eyes were examined at 1 day, 1 week, 1 month and 6 months postoperatively. Data on corrected visual acuity and surgically induced astigmatism values calculated by use of Cravy method were analyzed between two groups using Student's t and Chi-Square tests. Eyes in 4 mm sclerocorneal tunnel group displayed significantly better corrected visual acuity at the end of the first week postoperatively (p<0.05), and less surgically induced astigmatism at the end of the first and sixth month postoperatively (p<0.05, p<0.001 respectively). At the first month control no significant difference was observed in the visual acuity of the two groups. Peroperative and postoperative complications were few and the rate of complications was not significant between the two groups (p>0.05). As a result, both procedures of small incision cataract surgery offered satisfactory clinical results, but 4 mm sclerocorneal tunnel cataract surgery allowed significantly earlier recovery of visual function and less surgically induced astigmatism.
