We have examined 73 operated diabetic eyes and 70 eyes of nondiabetic matched controls who underwent ECCE with PC-IOL implantation over a two-year period. The diabetic group had a greater incidence of postoperative anterior segment complications. The major complications were appearence of fibrin (p<0.05),pigment dispersion (p<0.05), development of posterior synechiae (p<0.05) and desantralization of pupil (p>0.05). Transient iridal vascular congestion in 2 patients and iris neovascularization in an eye were observed. Older patients were found to be at greater risk for development of posterior syneshiae (p<0.005). Pigment dispersion was seen more often in patients with systemic hypertension (p<0.05) and prolonged diabetes (10 years or more). We suggest that diabetic patients who have undergone cataract surgery and PC-IOL implantation be seen at frequent intervals during the first postoperative weeks. Keywords: Diabetes mellitus, extracapsular cataract
