Prognostic Importance Of Filtration Bleb Aftertrabeculectomy Combined With Mitomycin C

* Uz.Dr.,SB Ankara Hastanesi Göz Kliniği,** Uz.Dr.,SB Ankara Hastanesi Göz Kliniği, Şefi, ANKARA To clarify the relation between the type of filtration bleb and intraocular pressure (IOP) decrease and success, in trabeculectomy combined with mitomycin C (MMC) applied eyes.
Patients and Method: Trabeculectomy operation was performed with intraoperative MMC application to 46 eyes of 44 patients with high risk glaucoma. Mean age was 40.4 (between 1-80 years of age), and mean follow-up period was 2.4 years (range 1-6 years). Surgery was performed with standard limbus based trabeculectomy technique. A rectangular scleral flap with dimensions 3x4 mm and 1/2 thickness was prepared. MMC solution was absorbed to a small surgical sponge and applied beneath the scleral flap for 5 minutes.
Application of trabeculectomy combined with MMC was found to be successful at 36 of 46 eyes (78.3%) after 2.4 years of follow-up. Mean IOP decrease of the study grup was; 21.03±6.89 mmHg in cases with avascular bleb, and 10.7±2.45 mmHg in cases with vascular bleb. At the end of the follow-up period, 91.7 % success was obtained among cases with avascular bleb, but this ratio was remained around 30 % among cases with vascular bleb.
Avascular bleb formation in high risk glaucoma cases as a result of trabeculectomy combined with MMC application was found to have prognostic importance.Keywords: Trabeculectomy, Glaucoma surgery,
