The Efficacy And Safety Of Prophylactic Heparin In The Prevention Ofexperimental Pupillary Fibrin Membrane Formation

Materials and Methods: Eighteen pigmented rabbits weighing 2000-2500 g were used in this study. One eye was taken from each animal. They were divided into two groups; control and heparin groups. In this study, at first 0.2 ml citrated blood sample was taken from ear vein and 0.2 ml aqueous humor was taken from anterior chamber from both groups. Then pupillary fibrin membrane formation was induced by administration of 0.2 ml citrated blood sample into anterior chamber of control group. Prophylactic 2 IU/0.2 ml heparin was administered into anterior chamber of heparin group and 30 minutes later citrated blood sample was given into anterior chamber. After 24 hours of giving citrated blood sample into anteror chamber, amount of pupillar membrane was graded by biomicroscopical examination at 0, 3, 6, 24, 48, 72 and 168 hours. The animals were also followed one week post-treatment in terms of intraocular pressure, corneal edema, inflammation of anterior chamber and lens opacity.
Result: When the efficacy of heparin in the treatment of pupillary membrane was compared to the control group, statistically significant differences were found at 0 h (p<0.01), 6 h (p<0.01), 24 h (p<0.001) and 168 h (p<0.01) after treatment. Side effects such as increased intraocular pressure, corneal edema, inflammation of the anterior chamber and lens opacity were not observed.
Conclusion: In this experimental pupillary fibrin membrane model, prophylactic 2 IU/0.2 ml heparin administered into the anterior chamber was found to be efficient in prevention of pupillary fibrin membrane formation. Pupillary fibrin membrane could be prevented by adding heparin into surgical irrigation fluid in patients without bleeding diathesis and who had a pupillary fibrin membrane formation tendency in anterior chamber.Keywords: Pupillary fibrin membrane, Anticoagulant agent,
