Evaluation Of Corneal Topography Maps With Numeric Indices In Patients With Type 2 Diabetes

* Yrd.Doç.Dr., İnönü Üniversitesi Tıp Fakültesi Göz Hastalıkları AD, MALATYA Purpose: We aimed to evaluate topographic corneal images quantitatively with numeric curvature indices in type 2 diabetic patients.
Materials and Methods: Computerized videokeratographs of 168 eyes of 88 patients with type 2 diabetes was studied and compared to those from both eyes of 30 control subjects with similar sex and age distribution. The diabetics with regulated blood glucose were survivors of Endocrinology Clinic of İnönü University School of Medicine. The diabetic patient sample according to the severity of retinopathy was divided into three subgroups; patients with no retinopathy, background retinopathy and proliferative retinopathy. Simulated Keratometry readings (Sim K1, Sim K2), Differential Sector Index (DSI) and Opposite Sector Index (OSI) which are sensitive to a localised peripheral corneal steepening were calculated to compare the topography maps objectively.
Results: No significant differences were detected in dioptric powers of central cornea (Sim K1 and Sim K2) in the control and diabetic groups (p=0.785, p=0.873 respectively). While the values of DSI in 4 groups did not differ significantly (p=0.128), OSI values were found to be different among the groups (p=0.037). In pairwaise comparisons between the groups, this difference was seen to result from corneal topographic changes in proliferative diabetic retinopathy group (p<0.05, p<0.05 and p<0.05). The correlations of OSI values with duration of diabetes and fasting blood glucose levels were not significant in diabetic patient sample (r=0.438, p=0.295 and r=0.161, p=0.823 respectively).
Conclusion: Our results suggest that changes in curvature may occur in paracentral and peripheral cornea in patients with type 2 diabetes mellitus. These corneal topographic changes seen in type 2 diabetic patients was found to be attributable to the severity of diabetic retinopathy rather than age, blood glucose and duration of diabetes.Keywords: Type 2 diabetes, Corneal topography,
