Apparent Accomodation After Cataract Extraction In Children

Göz Kliniği, Şefi, Ankara It is assesed that, some aphakic and pseudophakic patients have good near vision while wearing only their distance correction, a phenomenon called apperent accommodation. This study was designed to document the frequency and to explore the mechanisms of apparent accomodation in pediatric patients.
Apparent accomodation was evaluated in 12 eyes of 12 children who underwent cataract extraction with/without lens implantation for traumatic cataract. We did complete eye examinations in all cases. Near vision and near point of each patient was measured, and same measurements were done with an add of 3.0 diopter plus lens. Near vision without an add was considered excellent if it was within 2 lines of near vision with an add. It was considered good if it was within 3-5 lines of vision with an add. Near vision examinations were repeated after mydriasis with fenilefrin of 10% to evaluate the effectiveness of pupil size on apparent accomodation.
One patient was aphakic, 11 were pseudophakic. 9 eyes had excellent and 3 had good near vision. Apparent accomodation in aphakic/pseudophakic patients was decreased from 6.25 diopters to 5.04 diopters after mydriasis.
Our study represented that most aphakic and pseudophakic children had good vision at near without an add. Childeren old enough to be tested for near vision may be spared the sitgma of wearing bifocals.Keywords: Aphakia, Pseudophakia, Apparent accomodationTurkiye Klinikleri J Ophthalmol 2000, 9:149-152
