Ocular Adnexal Lymphoma With Lacrimal Gland Involvement

Purpose: To evaluate the demographic features, laterality of the lesions, signs and symptoms, associated systemic disease, treatment modalities, and outcome of patients with lacrimal gland lymphoma.
Material-methods: All patients at the Mayo Clinic, Rochester, Minnesota who had biopsy-proven lacrimal lymphoma between the years 1970-1997 were retrospectively evaluated.
Results: Of the 45 patients with lacrimal gland involvement, 21 (%47) had only lacrimal gland lesions while the remaining 24 (%53) had involvement of both lacrimal gland and other adnexal sites (conjunctiva, eyelid, orbital soft tissue other than lacrimal gland). The average age of 21 women and 24 men was 62.3 ± 13.2 years. Average duration of follow-up was 6.5 ± 5.2 years (median, 4.3 years). 32 patients had unilateral involvement and the remaining 13 had bilateral disease. Of the 45 patients with lacrimal gland lymphoma, 6 had prior documented systemic lymphoma, 11 had concurrent systemic involvement, and 26 had localized ocular disease. Systemic work-up results were not available for 2 patients. Of the 26 patients with localized lacrimal gland lymphoma, 9 (%35) developed subsequent systemic lymphoma after a median of 4.8 years (range, 0.6-17.3 years). Treatment consisted of surgical removal of the gland, radiotherapy, chemotherapy, combination of both radio- and chemotherapy, or observation. Ocular response to different treatment modalities was considered complete in 38 patients (84 %), incomplete in 6 (13 %), and was not available in 1 patient. Of the 45 patients 9 (20%) died due to lymphoma.
Conclusion: Ocular manifestations of ocular adnexal lymphoma with lacrimal gland involvement are managed successfully, however, these patients should be followed at appropriate intervals to identify possible future systemic lymphoma.
