Institution: Department of Refractive Surgery, Cerrahpaşa Medical School, Istanbul.
Materials and Methods: A retrospective study planned of 35 eyes of 28 patients, who had undergone H-LASIK treatment with L-type disc and Axicon system. The mean age of 23 males and 5 females was 35.4 years (22 to 57). Patients who were under 22 years old and lack of refractive stability at least for last one year, did not included to the study. 23% of the patients were presbiopic, and 56% of them had a spectacle corrected visual acuty under 0.5 before the H-LASIK treatment. In all patients H-LASIK performed using Apex Plus excimer laser, after preparing a 9 mm standard superior linged flap by Hansatome microkeratome. L-type disc was used in each case with optical zone of 6.5 mm, together with an Axicon optic extending blended zone to 9 mm. Patients were checked one day one week and 1, 3, 6, 9 months and a year after LASIK treatment.
Results: Mean baseline spherical equivalent manifest refraction of the patients was +4.985 ±1.93 D (range +1.625 to +9.125). One year after LASIK, mean manifest spherical equivalent refraction was +2.746±2.71 D. Refractive results were generally stable 9 months after LASIK. The predictability (to be in the range of ±1 D) of the refractive results were higher in the eyes whose baseline mean spherical equivalent refraction were under +4.0 D (31.4%). Only in one case who was complicated with epitelial ingrowth irregular astigmatism occurred and visual acuity reduced. No other complications were determined during the follow-up time. Uncorrected near visual acuity improved in 23% of cases and distant visual acuity was better in 52% of the eyes.
Conclusion: LASIK using erodible mask and Axicon system appears to be an effective and safe procedure to correct hyperopia up to +4.00 D.Keywords: Axicon optic, erodible disk, hyperopic laser in situ keratomileusis (H-LASIK)Turkiye Klinikleri J Ophthalmol 2001, 10:88-92
