Comparison Of Results Of One-site Versus Two-site Approaches At Combined Phacoemulsification And Trabeculectomy

Purpose: To compare the clinical results of one-site versus two-site approaches of combined phacoemulsification and trabeculectomy and intraocular lens (IOL) implantation techniques.
Methods: Eighty five eyes of 77 cases with a diagnosis of coexisting glaucoma and cataract and having an indication of surgical management were operated with a technique of combined phacoemulsification and trabeculectomy (phacotrabeculectomy) and IOL implantation. Cases were randomly assigned into two groups and the first group consisting of 51 eyes was operated by one-site approach (using the same incision for trabeculectomy, phacoemulsification and IOL implantation) while the second group consisting of 34 eyes was operated by two-site approach (using separate incisions for trabeculectomy, phacoemulsification and IOL implantation). Mean follow-up was 14.45 months. Two groups were evaluated according to age, sex, follow-up period, glaucoma type, preoperative and postoperative intraocular pressure, visual acuity and mean topical antiglaucoma drug use.
Results: Age, sex, follow-up period, glaucoma type distribution and preoperative values were similar in the two groups. At the last postoperative control, the mean intraocular pressure was significantly decreased and the mean visual acuity was significantly increased when compared with the preoperative values, but not significantly different between the two groups.One-site approach group required significantly more medications than did the two site group.
Conclusion: We recommend to use the two-site technique for combined phacotrabeculectomy and IOL implantation.
