The Effects Of Duration Of The Disease, Systemic Activation And Antirheumatoid Treatment On The Tear Function Tests And Conjunctival Impression Cytology In Patients With Rheumatoid Arthritis

Materials and Methods: Fourty five patients with rheumatoid arhtritis and thirty control cases was studied. Routine eye examination, Schirmer-I test, tear break-up time (BUT) test and impression cytology was performed to all cases. Erythrocyte sedimantation rate (ESR), C-reactive protein (CRP) and rheumatoid factor levels were determined. Anti-rheumatoid drug usage was also evaluated. Result: Mean Schirmer-I test result in patients with rheumatoid arhtritis was 9.03±5.42mm and in control cases 23±8.55mm (p<0.001). BUT in patients with rheumatoid arhtritis was 9.9±4.8 sn and in control cases 15.2±18sn (p<0.05). The rate of conjunctival epithelial squamous metaplasia were 53.33% in patients and 10% in control cases (p<0.05). There was a correlation between mean illness duration and Schirmer-I test, BUT and conjunctival cell structure. (respectively p<0.001, p<0.001, p<0.05). There was a correlation between rheumatoid arhtritis titer and Schirmer I, BUT and conjunctival cell structure (p<0.001), but no correlation was found between rheumatoid arhtritis titer and CRP, ESR (p>0.05). The tear function test results in patients with at least 6 months anti rheumatoid drug usage were similar to the control group results (p>0.05).Conclusion: We concluded that disease duration, inflamatory episode of the disease and anti-rheumatoid treatment were the basic variables that affected the tear function in patients with rheumatoid arhtritis.Keywords: Rheumatoid arhtritis, Tear function tests, Conjunctival impression cytology, Duration of the disease, Rheumatoid factor Anti-rheumatoid treatmentTurkiye Klinikleri J Ophthalmol 2002, 11:195-200
