Silicone Intubation With The Ritleng Method In Congenitalnasolacrimal Duct Obstruction

The mean age of the patients was 30 months (range, 9 months to 7.5 years). The Ritleng metal probe was advanced through the obstruction and into the inferior meatus. The prolene leader was introduced into the probe and advanced into the nasal cavity. The prolene leader emerged from the nose spontaneously or it was pulled out by using a metal hook. The same procedure was repeated through the other punctum. The tubes were fixed to the lateral nasal wall by a suture. Silicone tube removal was planned three months after this procedure. The mean follow-up time was 8.8 months (range 2 to 17 months) after tube removal.Results: Silicone intubation lasted 23 minutes, as avarege, (range, 15 to 35 minutes) for each eye. Both prolene leaders spontaneously emerged from the nose in only 2 eyes (10%). The use of hook was required in the other eyes. The most frequent postoperative complication was prolapse of the silicone tube in the medial canthal region and it occured in 5 eyes (24%). After tube removal, signs and symptoms of nasolacrimal duct obstruction was resolved completely in 19 eyes (90%), but not in two eyes.Conclusion: The Ritleng method makes silicone intubation procedure easier in children. In this method, the prolene leaders usually do not spontaneously come out from the nose.
