Risk Factors For Clinically Significant Macular Edema In Patients With Diabetic Retinopathy

Göz Kliniği Şefi, İSTANBUL Objective: To determine the risk factors for clinically significant macular edema (CSME) in patients with diabetic retinopa-thy (DR).

Materials and Methods: 1576 eyes of 788 patients attending to our clinic due to DR were retrospectively analyzed. 608 eyes of 382 patients with CSME were named as study group; whereas, 812 eyes of 406 patients without CSME were taken as control group. Age, gender, type of diabetes, age at the onset of diabetes, duration of diabetes, type of diabetes treatment, hemodialysis, hypertension (HT), hemo-globin A1c (HbA1c) level and degree of DR were the fac-tors which were evaluated for their effects on CSME. The multivariate-adjusted association between these factors and CSME was evaluated by logistic regression analysis. Chi-square and Student-t tests were used to investigate the inde-pendent effects of these factors on CSME.
Results: As analyzed by multiple logistic regression model, HbA1c level and degree of DR were found to be the statisti-cally significant factors for CSME (p<0,05); whereas, the effects of age, gender, type of diabetes, age at the onset of diabetes, duration of diabetes, type of diabetes treatment, hemodialysis and HT on CSME were found to be statisti-cally insignificant (p>0,05). On the other hand, when the factors were investigated independently, non-proliferatif DR and high HbA1c level were the statistically significant fac-tors for CSME (p<0,05); whereas, age, gender, type of dia-betes, age at the onset of diabetes, duration of diabetes, type of diabetes treatment, hemodialysis and HT did not have a statistically significant effect on CSME (p>0,05).
Conclusion: The degree of DR and high HbA1c levels are the significant risk factors in the development of CSME. Be-cause of the fact that most of the patients with CSME have background DR, it should be remembered during routine ophthalmologic examinations that CSME can be present in patients with newly diagnosed diabetes or background DR.Keywords: Diabetes, Clinically significant macular edema, Diabetic retinopathyTürkiye Klinikleri Oftalmoloji, 2003, 12:133-138
