Effects Of Posterior Capsular Disruption On The Outcome Of Phacoemulsification Surgery

Objective: To analyse the effects of posterior capsule (PC) rupture developed during phacoemulsification procedure on surgical success.
Materials and Methods: Prospective data were collected on cases developed PC rupture during phacoemulsification of 416 eyes of 373 consecutive patients between May 2000 and January 2002. The patient's age, preoperative visual acuity, ocular comorbidity, operative complica-tions, intraocular pressure, postoperative findings were documented. The best spectacle corrected visual acuity was recorded at the second month visit.
Results: Forty five patients (49 eyes) having pc rupture were consisted of 24 women and 21 men. The age of the pa-tients was between 45 and 91 years (68.3±9.2 years). The type of cataract was mature in 18 eyes, corticonuclear in 14 eyes and posterior subcapsular + nuclear in 17 eyes. Pc rupture occurred during the phacoemulsification in 28 eyes, cortex removal in 9 eyes, iol implantation-manipulation in 7 eyes, epinucleus extraction in 3 eyes and hydrodisection in 2 eyes. Pmma iol was implanted in 18 eyes and foldable iol in 28 eyes. In three eyes secon-dary iol implantation was performed. Good pupillary di-lation was not obtained in 13 eyes. Thirteen eyes had pseudoexfolation syndrome. Best corrected visual acuity in the second month was < 0.5 in 6 (%12.3) eyes, ≥0.5 in 43 (%87.7) eyes, >0.8 in 26 (53%) eyes.
Conclusion: Phacoemulsification surgery, even if complicated by pc rupture, is compatible with good visual outcome, if prompt attention is paid to the management of complica-tions.
