Results Of Combined Viscocanalostomy And Phacoemulsification

Materials and Methods: Seventeen eyes of 15 patients that underwent combined viscocanalostomy and phacoemulsification were enrolled in the study. Best corrected visual acuity level (BCVA), intraocular pressure (IOP) values, number of antiglaucoma medications used, visual field, anterior and posterior segment examination results were recorded preoperatively and postoperatively. Intraoperative and postoperative complications were determined. The surgery was considered a complete success with an IOP of less than or equal to 21mmHg without additional medication or surgery. A qualified success was defined as an IOP of less than or equal to 21mmHg with glaucoma medication. Improvement of BCVA by two or more standart Snellen lines was considered as a satisfactory visual outcome.Results: The mean preoperative IOP was 23.11±6.18 mmHg (14- 38mmHg). The patients were followed up for 14.11 +/- 2.17 months (12-17 months). IOP value decreased 39.2% at 1 month, 38.9% at 3 months, 38.1% at 6 months and 34.1% at 12 months postoperatively (p< 0.0001). Complete success rate was 82.3% and qualified success rate was 94.1% at 1 month postoperatively. These rates were 64.7% and 94.1% respectively at 12 months postoperatively. The number of antiglaucomatous medications used was significantly decreased at every follow up visit (p< 0.0001). In 16 of 17 eyes (94.1%), BCVA improved BCVA by two or more standart Snellen lines. There was not any significant intraoperative complication. Postoperatively, choroidal detachment was diagnosed in one eye and intraocular lens decentration was developed in another eye.Conclusion: Combined viscocanalostomy and phacoemulsification is safe and effective in lowering IOP and improving visual acuity.Keywords: Viscocanalostomy, Phacoemulsification, Combined surgery, Intraocular pressure, Visual outcomeTurkiye Klinikleri J Ophthalmol 2003, 12:102-107
