The Development Of Glaucoma After Penetrating Keratoplasty And Risk Factors

Ankara Eğitim ve Araştırma Hastanesi Göz Kliniği Şefi, ANKARA Objective: To evaluate the frequency and risk factors for the development of glaucoma after penetrating keratoplasty. Materials and Methods: Eight hundred and forty-seven penetrating keratoplasties (PKP) withouth preoperative glaucoma were analyzed retrospectively. The incidence and risk factors for the development of postkeratoplasty glaucoma were investigated.Results: Seventy three of 847 patients (8.6%) developed glaucoma after PKP. 73 patients were analyzed for various preoperative and peroperative conditions which may be related to the development of postkeratoplasty glaucoma: 15 eyes had inflammation during the PKP. Regrafts were performed in 5 eyes. Five patients had structural abnormality of anterior segment due to trauma. At the time of PKP, extracapsular cataract extraction posterior chamber intraocular lens implantation in 13 eyes, intraocular lens exchange, or removal or secondary placement of intraocular lenses in 8 eyes, and vitrectomy in 6 eyes were performed. There wasn't any risk factors for developing the glaucoma in 21 eyes. When patients were grouped according to the type of surgical procedure performed, the lowest incidence (3.5%) in patients undergoing first penetrating keratoplasty alone and the highest incidence (25%) of postkeratoplasty glaucoma in patients undergoing PKP with vitrectomy were seen. Conclusion: We concluded that combined surgical procedure with PKP, structural alteration of anterior segment due to trauma, inflammation during the penetrating keratoplasty and regraft might be risk factors for developing glaucoma after penetrating keratoplasty.Keywords: Penetrating keratoplasty, Glaucoma, Combined keratoplastiesTurkiye Klinikleri J Ophthalmol 2003, 12:37-41
