The Effect Of Phacoemulsification Surgery On Intraocular Pressure In Trabeculectomised Eyes

Dr., Ege Üniversitesi Tıp Fakültesi Göz Hastalıkları AD,*** Prof.Dr., Ege Üniversitesi Tıp Fakültesi Göz Hastalıkları AD, İZMİR Objective: To evaluate the effect of phacoemulsification surgery on intraocular pressure (IOP) in eyes that underwent prior trabeculectomy.Institution: Ege University School of Medicine Ophthalmology Department Materials and Methods: Thirty- two patients who underwent phacoemulsification subsequent to trabeculectomy were included in the study. All patients had primary open angle galucoma and have been followed up regularly. Phacoemulsification procedures were performed by the same surgical team with the same technique including temporal clear corneal incision.Results: Subsequent phacoemulsification was performed 52.23±45.07 months (3-216 months) after trabeculectomy. Follow- up period of the patients after cataract extraction were 22.62±12.22 (14-72 months) months. While mean IOP was 15.03±4.63 mmHg (8-27 mmHg) with 0.56±0.84 medications prior to phacoemulsification, it increased to 13.84±3.74 mmHg (8-22 mmHg) with 0.5±0.76 medications (0-2 medications) at 1 month and 16.28±3.92 mmHg (8-27 mmHg) with 0.75±1.01 medications (0-4 medications) at 12 months after the cataract surgery. Postoperative visual acuity did not change in 6 cases, in the remaining 26 cases the increament of visual acuity was ranged from 1 to 6 Snellen lines.Conclusion: Phacoemulsification had an adverse effect on IOP in eyes that underwent prior trabeculectomy. Although this effect was far from neutralising the pressure lowering effect of prior trabeculectomy, IOP and the number of medications used increased beginning from first post- operative month.Keywords: Trabeculectomy, Phacoemulsification, Intraocular pressureTurkiye Klinikleri J Ophthalmol 2003, 12:42-47
