Long-term Results Of Primary Trabeculectomy In Cases With Thin Tenon's Capsule And Conjunctiva

* Arş.Gör.Dr., Gazi Üniversitesi Tıp Fakültesi Gazi Hastanesi Göz Hastalıkları AD,** Prof.Dr., Gazi Üniversitesi Tıp Fakültesi Gazi Hastanesi Göz Hastalıkları AD, ANKARA Objective: To evaluate the long-term results of primary trabeculectomy without antiproliferative agents in cases who were older than 40 years and had thin Tenon's capsule and conjunctiva.Institution: Department of Ophthalmology, School of Medicine, Gazi University.Material and Methods: Fifty-five eyes of 55 cases without high-risk characteristics operated between 1.12.1992 and 1.05.1998 were evaluated retrospectively. All cases were older than 40 years of age and they had thin Tenon's capsule and conjunctiva determined intraoperatively. Surgical success was achieved if intraocular pressure was lower than 21 mm Hg with or without anti-glaucomatous medications.Results: Surgical success was 86.5% after 1-year follow-up, it decreased to 81.9% after 3 years. The 5 year success rate was also 81.9%. At last visits, 81.8% of the cases had the same or better (2 or more Snellen's lines increase) visual acuity and 83.6% of the cases had type-1 or type-2 functional filtrating blebs. Complications observed after trabeculectomy were as follows: cataract progression (14 cases), shallow anterior chamber (7 cases), encapsulated bleb (3 cases), hypotonous maculopathy (1 case) and endophthalmitis (1 case).Conclusion: Over 40 years of age, primary trabeculectomy without antiproliferative agents may have high long-term success rate in cases with thin Tenon's capsule and conjunctiva without high-risk characteristics.Keywords: Glaucoma, Primary trabeculectomy, Tenon's capsule, Long-term follow-upTurkiye Klinikleri J Ophthalmol 2003, 12:1-8
