Identify And Determine Antibiotic Susceptibility Of Coagulase Negative Staphylococci Isolated With Chronic Blepharitis, Ocular Rosacea And Dry Eye

Objective: To identify and determine antibiotic susceptibilitiy of coagulase negative staphylococci (CoNS) isolated from patients with chronic blepharitis, ocular rosacea and dry eye.
Material and Methods: A prospective review of all positive CoNS cases of chronic blepharitis, ocular rosacea and dry eye between June 2003-March 2004 was performed. Species identification and antibiotic susceptibility test were performed by using Auto Sceptor TM.
Results: Of the 219 CoNS, 170 S.epidermidis, 19 S.warneri, 13 S.capitis, 7 S.hominis, 5 S.xylosis, 2 S.simulans, 2 S.lugdunensis, 1 S.equorum were observed. The culture positivity rates of S.epidermidis in chronic blepharitis, ocular rosacea, and dry eye were found 77.7%, 72.7%, and 87.5% respectively. One hundred fifty four (70%) CoNS were penicillin, 127 of them (58%) were tetracyclin, 48 (22%) gentamycin, 29 (13%) trimethoprim-sulfamethoxazole, 20 (9%) erythromycin, 14 (6%) ciprofloxacin, 13 (6%) levofloxacin, and 8 (4%) oxacillin resistant. In total, 29 strains were resistant to three or more antibiotics. One hundred twenty eight S.epidermidis strains were resistant to penicillin, 95 (56%) to tetracyclin, 37 (22%) to gentamycin, 24 (14%) to trimetoprim-sulfomethoxazole, 13 (8%) to erythromycin, 12 (7%) to ciprofloxacin, 10 (6%) to levofloxacin and 8 (5%) to oxacillin. No differences was observed among antibiotic resistance in all strains tested (p> 0.05).
Conclusion: CoNS, especially S.epidermidis strains are dominant members of the lid and conjunctival flora. Since the antibiotic susceptibility of S.epidermidis is unpredictable and multiresistant strains are common, we recommend that antibiotic susceptibility testing be performed in all cases of ocular infections caused by these microorganisms.
