Congenital Ptosis: Characteristics Of Refraction And Amblyopia

Objective: To evaluate reasons for amblyopia in congenital ptosis.
Material and Methods: Medical records of 34 patients with congenital ptosis who were examined in Ophthalmology Department of Dokuz Eylul University between April 1997 and April 2002 were retrospectively rewieved.
Results: Thirty four cases, 13 (%38,2) female, 21(%61,8) male with an average age of 89 months were included in the study. Thirty two patients had unilateral and 2 patients had bilateral symetrical congenital ptosis. Marcus Gunn Jaw Winking Syndrome was found in 5 (%14,7) patients. Amblyopia were examined in 10 (29,4%) patients. In 9 of these patients, ptosis was unilateral and severity of ptosis was as follows: 3 mild, 3 moderate, 3 severe in these patients. Amblyopia were examined in one of the patients with bilateral ptosis. Myopic astigmatism and strabismus were amblyogenic factors in this patient. In 80 % of patients with amblyopia, anisometropia was determined in our study. Only 2 of the isometropic patients were amblyopic. One had deprivation amblyopia and the other had Marcus Gunn Jaw Winking Syndrome. No amblyogenic factor could be found in the latter case. Anisometropia were found in 12 (35,2%) of the patients and 8 of them had amblyopia. Six (17,6%) of the patients were strabismic and strabismus was found to be a ambliogenic factor in 3 of them. Corpus callosum agenesis was examined in a patient with moderate ptosis accompanied by exotropia. Superior rectus hypofunction accompanied ptosis in 5 (14,7%) patients. Superior rectus hyperfunction was found with in one patient.
Conclusion: Ptosis severity was not the only factor for amblyopia in congenital ptosis, but also astigmatism related with ptosis and other additional factors were postulated as the factors for amblyopia in this clinical problem. Related pathologies like anisometropia, high astigmatism, strabismus and ambliopia should be evaluated in patients with congenital ptosis.
