Histopathology Of M. Levator Palpebrae Superioris In Thyroid-related Upper Eyelid Retraction

Levator Palpebrae Superioris (levator muscle) in thyroid-related upper lid retraction in order to collect some evidence of pathogenesis.Material and Methods: Six samples of levator muscle were obtained from patients undergoing surgery for thyroid-related upper lid retraction. Specimens were stained with Haematoxylin Eosin, Verhoeff-van Gieson and Mallory-Azan, and observed under light microscope. Results were compared with the specimen obtained from a patient operated for post-traumatic uper lid retraction.Results: Striated muscle fibers were infiltrated by fatty tissue. These fibers were surrounded by thick endomysium and showed varying degrees of atrophy. Bundles of muscle fibers were in a dense fibrotic connective tissue, mostly devoid of elastic fibers. The post-traumatic case demonstrated a collagen-rich connective tissue, and normal striated muscle fiber structure with only minimal fat infiltration.Conclusion: Fibrosis, decreased elasticity of the connective tissue, fatty infiltration of the levator muscle, along with atrophy and endomysial hypertrophy of the muscle fibers may each play a role in the pathogenesis of thyroid-related upper lid retraction.Keywords: Upper lid retraction, thyroid orbitopathy, m. levator palpebrae superioris, histopathologyTurkiye Klinikleri J Ophthalmol 2004, 13:124-128
