Levator Aponeurosis And Mueller's Muscle Recession In Thyroid Related Upper Eyelid Retraction

Göz Kliniği Şefi, ANKARA Objective: To evaluate the effectiveness of graded levator aponeurosis and Mueller's muscle recession in upper lid retraction related to thyroid orbitopathy.Material and Methods: Ten consecutive patients' 12 eyelids were operated with the same technique, incorporating a levator aponeurosis and Mueller's muscle recession with maintenance of the normal orbital septum-levator aponeurosis anatomy. The amount of recession was assessed per-operatively. Mean follow?up was 8.1 ± 2.4 months. A Margin ? Reflex Distance (MRD) of 4.5 ± 1.0 mm, and a maximum asymmetry of 1.0 mm in lid levels and lid crease (LC) heights at the last visit were defined as successful results.Results: A mean drop of 3.1 mm in lid levels was achieved; with a corresponding mean elevation of 2.6 mm in LC heights. Mean final MRD was 3.6 mm. Eight patients achieved symmetrical results, while two had asymmetrical lid levels and LC heights. Conclusion: Levator aponeurosis and Mueller's muscle recession tailored to the individual patient intra-operatively, can be used for different levels of lid retraction.Keywords: Upper lid retraction,Thyroid orbitopathy, Levator aponeurosis and Mueller's muscle recessionTurkiye Klinikleri J Ophthalmol 2004, 13:82-87
