Tear Level Of Leukotrien B4 After Plannedextracapsular Cataract Surgery

aGöz Hastalıkları AD, bMikrobiyoloji ve Klinik Mikrobiyoloji AD, Celal Bayar Üniversitesi Tıp Fakültesi, MANİSA Objective: To evaluate the level of leukotriene B4 (LTB4) in human tears and the statistical relation between the degree of post-operative inflammation and the presence of LTB4 after extracapsular cataract extraction and intraocular lens implantation (ECCE+IOL).
Material and Methods: The study population consisted of 16 patients undergoing ECCE+IOL. Patients with diabetes mellitus and who were using eyedrops before the surgery were excluded. Tear samples were collected from the lower conjunctival fornix via a capillary glass tube before and after the surgery. The samples were stored at -20˚C until the analysis. Preoperative assessment of patients consisted of eye examination with grading the degree of meibomian gland function and ocular surface status. The level of clinical inflammation was recorded after surgery. The LTB4 levels were measured by using ELISA technique (Cayman Chemicals).
Results: The age of the patients was 65.3 ± 8.9 (mean ± standart deviation) years. There were 11 (68.8%) male and 5 (32.2%) female patients. LTB4 levels have been found to rise significantly in 4 of the operated eyes. There was a significant difference between LTB4 levels and post-operative inflammation scoring between the operated and the fellow eyes (p< 0.05, p< 0.02). The postoperative inflammatory status of the eyes who had positive tear LTB4 levels was not statistically different from the eyes without detectable tear LTB4 levels. (p> 0.05).
Conclusion: Intraocular surgery causes a rise of LTB4 levels in the tears of patients who has undergone an intraocular operation. The inflammatory status of the eyes with the detectable presence of LTB4 in tears do not differ from the inflammatory status of the eyes without detectable LTB4 levels and there is no concomitant rise of LTB4 in the fellow eyes.Keywords: Leukotriene B4, cataract surgery, tear filmTurkiye Klinikleri J Ophthalmol 2005, 14:1-6
