A patient with diplopia which developed after a traffic accident was diagnosed as an isolated inferior rectus palsy. He had 27 prism diopters(pd) of hypertropia in the primary position of gaze. The patient had surgery at which a total transposition of horizontal rectus muscles were performed. The residuel 14 pd of hypertropia was corrected to 5 pd by recession of the contralateral inferior rectus muscle. Keywords: Paralitic strabismus, Inferior rectus paralysis, Total transpositionTurkiye Klinikleri Oftalmoloji 1997, 6:206-209
