Fully Transpositions Of Horizontal Rectus Muscles At Double Elevator Palsy

aGöz Hastalıkları AD, Gaziantep Üniversitesi Tıp Fakültesi, GAZİANTEP Objective: In double elevator palsy (DEP), there are serious restrictions which cause not only abnormal head positions and also cause cosmetic problems. This study was planned to evaluate the effectiveness of the full tendon transpositions of the rectus muscles in DEP.
Material and Methods: Operation of horizontal rectus muscles transpositions were done in four DEP patients who were diagnosed, treated and followed up in the Department of Ophthalmology, Medical School of Gaziantep University.
Results: Mean age of four cases was 21 year (4-34 years). Elevation in both abduction and adduction has been achieved in all cases (100%). No restriction was observed to nasal, temporal or down gazes. Only in one patient (25%) consecutive exotropia has been developed. Although in two cases (50%) ptosis and abnormal head positions were improved, for one case surgery to ptosis and for other case surgery to the inferior rectus were planned.
Conclusion: In DEP cases with hypotropia that effected eye can not come to the midline, full tendon transpositions of the rectus muscles may be a choice, because of satisfactory results and minimum complications.Keywords: Fully tendon transposition, double elevator palsyTurkiye Klinikleri J Ophthalmol 2005, 14:147-150
