Scleral Sutured Posterior Chamber Intraocular Lens Implantation

Objective: To review the treatment indications and results of scleral-sutured posterior chamber intraocular lenses that were implanted in the absence of adequate posterior capsular support.
Material and Methods: Ten eyes of 10 cases that underwent scleral-sutured posterior chamber intraocular lens implantation at the Department of Ophthalmology, Ankara University Faculty of Medicine between October 2001 and August 2004 were retrospectively evaluated.
Results: Eight of ten patients were males and 2 were females. The mean age was 52 years, ranging from 24 to 79 years. Five eyes had a history of blunt or penetrating eye trauma. Four eyes were aphakic; 4 eyes had intraocular lens subluxation into the vitreous and the other 2 eyes had lens subluxation. The preoperative visual acuities ranged between counting fingers to 0.4. Four aphakic eyes underwent anterior vitrectomy and scleral-sutured posterior chamber intraocular lens implantation. Four eyes with intraocular lens subluxation into the vitreous underwent pars plana vitrectomy, floating of the intraocular lens to the anterior chamber using perfluorocarbon liquid, extraction of the dislocated intraocular lens via a corneal incision, and placement of scleral-sutured posterior chamber intraocular lens. Two eyes with traumatic lens subluxation underwent lens extraction, anterior vitrectomy and scleral-sutured posterior chamber intraocular lens implantation. The postoperative complications were transient corneal edema in 1 eye, glaucoma escalation in 1 eye, and uveitis in 1 eye. At a mean follow-up of 6 to 36 months, the final postoperative visual acuities ranged from 0.1 to 0.9.
Conclusions: We found scleral-sutured posterior chamber intraocular lens implantation to be a useful method for aphakic rehabilitation in 10 eyes with inadequate posterior capsular support. Long-term studies with a larger number of patients may better define the role of scleral-sutured posterior chamber intraocular lens implantation in our circumstances.
