Learning Curve In Phacoemulsification - ı ı : Comparison Of Two Surgeons

aGöz Hastalıkları AD, Başkent Üniversitesi, Adana Uygulama ve Araştırma Hastanesi, ADANA Objective: To compare the results of phacoemulsification cases of two surgeons, and thus investigate the number of needed surgery, by that the complication rate decreases.
Material and Methods: The data from the first 250 phacoemulsification cases of two surgeons were evaluated retrospectively. Patients were studied for age, sex, laterality of the operated eye, preoperative and postoperative best corrected visual acuity, nuclear hardness, presence of pseudoexfoliation, axial length, presence of posterior capsular rupture and its stage in the surgery, and the localization of the intraocular lens. The difference among surgeons and the difference of each surgeon in success in the course of time were evaluated statistically using Pearson chi-square and Kruskal Wallis tests. The correlation of complication rates with nuclear hardness and pseudoexfoliation was evaluated with Spearman’s correlation analysis.
Results: A male predominance was observed in the first group (p=0.025). No statistically significant difference for age, laterality, axial length, and postoperative best corrected visual acuity was observed between the groups (p>0.05). The posterior capsular rupture rate was 4.4% in surgeon 1, and 6.8% in surgeon 2, however the difference was insignificant (p=0.243). Capsular rupture was more common during irrigation-aspiration. The correlation for cataract hardness and presence of pseudoexfoliation with complication occurrence was not significant (p=0.145 and p=0.905, respectively). The complication rate in the course of time showed a decrease after 100 cases in both surgeons. This decrease was statistically significant in surgeon 2 (p=0.034).
Conclusion: In this present study a decrease in the complication rates after the first 100 phaco surgeries was observed. It was observed that, with the gain of experience and correct patient selection, the hardness of the cataract and presence of mild pseudoexfoliation did not increase the risk of complication.Keywords: Cataract, cataract extraction, phacoemulsification, pseudoexfoliationTurkiye Klinikleri J Ophthalmol 2005, 14:156-161
