Results Of Cataract Surgery ın Patients Who Had Previously Undergone Pars Plana Vitrectomy

aII. Göz Kliniği, SB Ulucanlar Göz Eğitim ve Araştırma Hastanesi, ANKARA Objective: To evaluate the results of cataract surgery in patients who had previously undergone pars plana vitrectomy (PPV).
Material and Methods: Seventy four eyes of the 74 patients who had cataract extraction after previous PPV were reviewed for the type of cataract, type of cataract surgical technique, reason for vitrectomy, use of intravitreal tamponade at the time of PPV surgery, intraoperative and postoperative complications and visual outcomes.
Results: There were 38 male (51%) and 36 female (49%) patients with a mean age of 52.7 years. The indications for PPV were; vitreous hemorrhage in 47 eyes (64%), retinal detachment in 49 eyes (26%), vitreous condensation secondary to uveitis in 2 eyes (3%), asteroid hyalozis in 3 eyes (4%), and intraocular foreign body (4%). Phacoemulsification was performed in 28 (38%) patients and extracapsular cataract extraction (ECCE) in 46 (62%) patients. Anterior chamber became extremely deep in the 21 (75%) patients in the phacoemulsification group. There were some difficulties at the time of nucleus expression due to hypotonia and lack of vitreous gel in the 27 patients (57.8%) in ECCE group. Posterior capsule rupture occurred in 7 (%25) eyes in phacoemulsification group and in14 (30%) eyes in ECCE group (p=0.615). Intraocular lens was implanted in 19 (68%) eyes in phacoemulsification group and in 34 (74%) eyes in ECCE group. Intraocular lens was not implanted in 9 (32%) and 12 (26%) eyes respectively (p=0.575). A dropped nucleus occurred in 2 (3%) eyes in ECCE group. Visual acuity improved in 51 (69%) eyes, maintained in 13 (17.5%) eyes, and worsened in 10 (13.5%) eyes.
Conclusion: The most common problems at the time of cataract surgery in previously vitrectomized eyes were extremely deep anterior chamber in phacoemulsification group and difficulties in the nucleus expression in ECCE group. Although, there is no statistically significant difference in terms of complications between the two groups, phacoemulsification seems to be a relatively safer procedure than ECCE for vitrectomized eyes.Keywords: Cataract, vitrectomy, phacoemulsification, intraocular lensTurkiye Klinikleri J Ophthalmol 2006, 15:25-30
