Relationship Between Pattern Dystrophy, Diabettes Mellitus, And Hearing Loss

aGöz Hastalıkları AD, Başkent Üniversitesi Tıp Fakültesi, ANKARA 54-years-old male patient who has been followed for diabetes mellitus (DM) for 4 years, was examined in our department because of the possible retinal complications. He didn’t have any ophthalmologic complaints and best-corrected vision was 20/20 in both eyes. Anterior segment slit-lamp examination revealed normal findings. Intraocular pressure levels were 15 mmHg in right eye and 14 mmHg in left eye. Fundoscopy revealed bilateral, symmetrical, multiple hypopigmented flat lesions around the foveal region. There were no signs of diabetic retinopathy. Fundus fluorescein angiography showed bilateral symmetrically distributed hypofluorescent lesions, which were surrounded by hyperfluorescent window defects in the foveal region. Visual field and Ishihara color vision tests were within normal limits. In accordence with these findings, the patient was diagnosed as pattern dystrophy. To evaluate deafness accompanying the dystrophy, audiometer test was applied to the patient. Although the test was in normal limits; decrease at low frequency and incisures at 4000 Hz was assessed.
Butterfly-shaped dystrophy is a rare condition, which is usually inherited as autosomal dominant. The diagnosis is based on the fundus examination and fluorescein angiography. When associated systemic findings like DM and deafness are discovered, mitochondrial point mutation should be thought.Keywords: Diabetes mellitus, hearing loss, pattern dystrophyTurkiye Klinikleri J Ophthalmol 2006, 15:31-34
