A Case Of Malignant Hypertension Diagnosed By Unilateral Visual Loss

aGöz Hastalıkları AD, Selçuk Üniversitesi Meram Tıp Fakültesi, KONYA A 19 years old female attended to our clinic with a complain of unilateral decreased vision noticed since 1 month. The patient denied any systemic disease, except intermittant headache since 1 month. Ophthalmological examination revealed a corrected visual acuity of 0.9 on the right eye and 0.4 on the left eye. Anterior segments were bilateraly normal while fundus examination showed edema of the optic disc, macular star appearance and diffusely expanding hyperpigmented and yellow spots in retina. In fundus fluorescein angiography, besides edema of the disc and macula, hyperfluorescence was observed in areas corresponding to yellow spots, in contrast to hypofluorescence observed in hyperpigmented areas of fundus. The systemic check-up required regarding these findings revealed a systemic pressure of 220/150 mmHg. The patient was consulted to nephrology clinic for a through investigation of the etiology, as plasma urea and creatinine levels were also increased.
A macular star appearance and disc edema should always remind malignant hypertension, especially if any sign of an inflammation is lacking and leopard spot appearance related to hypertensive choroidopathy is observed in retina.Keywords: Malignant, hypertension, visual loss, retina, choroidTurkiye Klinikleri J Ophthalmol 2006, 15:140-143
