Efficacy Of Occlusion Treatment And Factors Affecting The Success Of Treatment In Amblyopia

aGöz Hastalıkları AD, Ege Üniversitesi Tıp Fakültesi, İZMİR Objective: To evaluate the efficacy of occlusion treatment and factors affecting the success in amblyopic cases.
Material and Methods: Records of 107 pediatric patients who had unilateral amblyopia were retrospectively reviewed. The efficacy of age, level of visual acuity at the beginning of treatment, type of amblyopia, occlusion period and refractive errors on visual outcome and binocular vision were evaluated.
Results: Of 107 patients, 46 (42.9%) were female and 61 (57.1%) were male. The mean age was 5.8 ± 1.8 years (3 to 12 years) and the mean follow-up time was 60.3 ± 52.8 months (28 to 120 months). In eighty-six of our patients (%80.3), the visual acuity was improved at least 2 lines at the end of treatment. The mean visual acuity improvements was similar among the groups (strabismic amblyopia 0.43 ± 0.03 logMAR units, anisometropic amblyopia 0.5 ± 0.08 logMAR units, strabismic-anizometropic amblyopia 0.42 ± 0.08 logMAR units). Most of the improvement in visual acuity was accomplished within first 3 months. Besides, the improvements at 6th month, 1st and 2st year were statistically significant (p= 0.000, p= 0.000, p= 0.000, p= 0.000, respectively). When we analyze the results of all patients, we determined that the level of initial visual acuity, time of occlusion and age at the initial of treatment were the most significant factors on final visual acuity (p= 0.000, p= 0.000 p= 0.003, respectively). We also determined that sex, type of amblyopia, treatment duration, refractive error do not have on effect on the final visual acuity (p= 0.72, p= 0.66, p= 0.82, p= 0.88, respectively).
Conclusion: Occlusion of the sound eye is an effective treatment modality for amblyopia. Most of improvement in visual acuity was accomplished within first 3 months. The most important factors on the final visual acuity are level of initial visual acuity, time of occlusion and age at initiation of treatment.Keywords: Amblyopia; occlusion treatment; visual acuityTurkiye Klinikleri J Ophthalmol 2007, 16:1-8
