Topical Mitomycin C Aided Pterygium Surgery Without Excission

aGöz Hastalıkları AD, Mustafa Kemal Üniversitesi Tıp Fakültesi, HATAY Objective: To evaluate the efficacy of combined pterygium folding and intraoperative topical application of mitomycin C (MMC) technique in preventing recurrence, and to determine the complications related to this technique.
Material and Methods: In a prospective, non-comparative case series, 23 eyes of 20 consecutive patients with primary pterygium were studied. After limbal conjunctival incisions from each side of the pterygia, the pterygia were detached from the underlying cornea and sclera with the help of a crescent knife under subconjunctival anesthesia, and 0.02% MMC was applied topically for 2 minutes to the subconjunctival surface of pterygia after folding it inversely. Then, the pterygia were folded on itself, and healthy conjunctiva was sutured to the neighboring healthy conjunctiva.
Results: Mean age of the patients was 47.5 ± 12.4 years (range 30-70 years). Three eyes (13%) had grade 1, 17 eyes (74%) had grade 2 and 3 eyes (13%) had grade 3 pterygia. The mean follow up was 12.4 ± 3.9 months (range 8-22 months). No recurrence or sight-threatening complications were encountered in any of the patients throughout the follow-up.
Conclusion: Although folding pterygium surgery, aided with low dose mitomycin C application, without excision seems to be an effective surgical alternative technique by reducing surgical time and preventing recurrence of primary pterygium, comparative, randomized further trials with more number of patients and longer follow ups are required to establish the safety and efficacy of this treatment.Keywords: Pterygium; 1 alpha -(4-(N-succinimidoxycarbonyl) butyryl) mitomycin CTurkiye Klinikleri J Ophthalmol 2007, 16:145-149
