a2. Göz Kliniği, Ankara Eğitim ve Araştırma Hastanesi, ANKARA Objective: This study was conducted to evaluate success and failure reasons of external dacryocystorhinostomy (ext-DCR) performed in our clinic.
Material and Methods: We retrospectively reviewed 400 patients’ 453 eyes those had primer acquired nasolacrimal obstruction. Mean follow up time was 1 year. We evaluated success and failure reasons of external dacryocystorhinostomy performed in our clinics.
Results: Out of 453 ext-DCR, 43 of them were failed. In three of the failed ext-DCR, bicanalicular silicon intubation (BSI) and endoscopic excision of the granulation tissue which was detected in the edge of the ostium was performed. In one of the failed ext-DCR, BSI and endoscopic separation of adhesion between nasal septum and anastomosis were performed. In thirty nine failed cases because of inadequate location and dimension of ostium determined during intraoperative examination, DCR combined BSI was performed. Of the 43 failures, 41 success results were taken after second operations. Success rates after the first and second operation were 90.5% and 99.5 % respectively.
Conclusion: In terms of success rate, patient satisfaction and cost effectiveness, ext-DCR is the most effective treatment in nasolacrimal obstructions. Inadequate location and dimension of ostium are the most common reasons of failure. Keywords: Lacrimal duct obstruction; dacryocystorhinostomyTurkiye Klinikleri J Ophthalmol 2007, 16:159-162
