Multivariate Risk Factors Analysis In Inferior Rhegmatogenous Retinal Detachment Operated By Scleral Buckling Surgery

Material and Methods: 48 eyes of 48 patients with rhegmatogenous inferior retinal detachment operated by scleral buckling and with a follow-up time between 6-36 months were included in the study. A retrospective review of records was done. Exclusion criteria were giant retinal tears, retinal dialysis, proliferative vitreoretinopathy grade C or higher, schisis detachments and vitreous opacities. Preoperatively the mean number of retinal tears and detached quadrants were 2.5 ± 1.3 and 2.3 ± 0.5, respectively. In 41 eyes encircling buckle and in 46 eyes limbus parallel buckle were placed. In 33 eyes subretinal fluid drainage and in all eyes cryopexy were performed. SF6 was injected in 13 eyes. Potential risk factors influencing anatomic success (age, location of detached retina, macula-on/off, number of breaks, cryopexy, aphakia, pseudophakia, Nd-YAG-capsulotomy, capsular rupture with vitreous-prolapse, encircling-buckle, limbus parallel buckle, subretinal fluid drainage, subretinal hemorrhage, myopia≥ 6 D, SF6) were evaluated by using multivariate logistic regression.
Results: Retinal reattachment was achieved in 39 eyes (81.3%) after primary surgery. In 8 of 9 eyes with anatomic failure, after second step with pars plana vitrectomy retina was reattached. After the second step the overall success was 97.9%. Multivariate logistic regression analysis showed number of breaks (p= 0.036) and posterior capsular rupture with vitreous-prolapse (p= 0.003) as independent risk factors for adverse anatomic outcome.
Conclusion: Number of breaks and posterior capsular rupture with vitreous-prolapse are independent risk factors affecting the anatomic success of conventional scleral buckling surgery.Keywords: Scleral buckling; retinal detachment; risk factorsTurkiye Klinikleri J Ophthalmol 2007, 16:178-183
