Suprachoroidal Hemorrhage And Hyphema During Phacoemulsification In A Patient On Anticoagulant Therapy

aGöz Hastalıkları AD, Abant İzzet Baysal Üniversitesi, İzzet Baysal Tıp Fakültesi, BOLU Suprachoroidal hemorrhage (SCH) is a rare complication of phacoemulsification surgery. This report aims to present a case of suprachoroidal hemorrhage and hyphema during phacoemulsification surgery in a patient on anticoagulant therapy. Phacoemulsification under topical anesthesia was performed on the right eye of an 80-year-old woman with a history of myocardial infarction, hypertension, cardiac failure, diabetes, and anticoagulant drug usage. After the incision was closed with a single suture, shallowing of the anterior chamber, loss of the red fundus reflex, and hyphema were observed. The diagnosis of SCH was verified with ultrasonography. SCH and hyphema were observed to have entirely resolved without any intervention after 4 weeks postoperatively.
Despite the fact that the incidence of SCH in phacoemulsification under topical anesthesia is very low when compared with former surgical methods, this complication may occur, though rarely, in patients with some risk factors. Even though anticoagulant therapy is not a well-established risk factor for SCH associated with phacoemulsification, it would be advisable to keep in mind its potential for increasing the risk of hemorrhage.Keywords: Phacoemulsification, intraoperative complications, anticoagulantsTurkiye Klinikleri J Ophthalmol 2007, 16:207-211
