Astigmatism In Children With Inactive Graves’ Ophthalmopathy

aPediatrik Oftalmoloji Kliniği,bPediatrik Endokrinoloji Bölümü, Dışkapı Çocuk Hastalıkları Eğitim ve Araştırma Hastanesi, ANKARA Objective: To compare the prevalence of astigmatism in children with inactive Graves’ ophthalmopathy (IGO) with age and sex matched controls. Material and Methods: Fifteen children with IGO and 15 age and sex matched controls were included in this prospective study. Duration of Graves’ disease and ophthalmopathy, the level of TSH (thyroid stimulating hormone) receptor antibodies, the degree of exophthalmos and upper eyelid retraction were noted in the study group. Cycloplegic autorefraction was performed and ocular movements were documented and the restriction of ocular movement was graded in cases with ocular movement restriction. Groups were compared in terms of prevalence of astigmatism. The relationship between the parameters related with Graves’ disease and astigmatism was evaluated. Rank correlation test and generalized lineer estimating equations were used for statistical analysis. Results: The prevalence of astigmatism was 30% (detected in 9 of 30 eyes) in IGO group and 10% (detected in 3 of 30 eyes) in age and sex matched controls (p<0.02). In the 9 eyes with astigmatism in IGO group, elevation was restricted in 4 eyes of 2 children with astigmatism at meridians of 175-5 degrees, abduction was restricted in both eyes of one child with astigmatism at meridians of 85-95 degrees. In remaining 3 eyes with astigmatism at meridians of 95-100 (right eye of one child) and 100-110 (left eyes of two children) degrees, ocular movements were not restricted. Among the parameters related to Graves’ disease, a correlation was determined between only ocular movement restriction and astigmatisma. Conclusion: Graves' ophthalmopathy is associated with a higher prevalence of astigmatism. Among patients with Graves’ ophthalmopathy, prevalence of restriction of ocular movements is higher in eyes with astigmatism.
